Coverage and comparison limits
The counts above are from the August 2026 inventory. The selected subset must be checked against its hospital source files. Comprehensive commercial, outpatient, professional and cash-pay coverage is excluded.
A published rate is not an amount paid on a claim. Case rates, per-diem rates and other rate types are kept distinct. MA classification and service-code years require review.
Where supplied, a Medicare index uses a modeled baseline with stated components and exclusions. Source verification does not by itself establish contract comparability or a complete Medicare allowed payment.
